* When you go to enroll, the United Healthcare summary of benefits will show the full deductible, as the plans are utilized, another carrier (Manhattan Life) will step in to cover claims (up to 1/2 or a little more depending on the plan level) directly at time of service with NO claim forms.
** The Medical (United Healthcare) and GAP (Manhattan Life) plans run concurrently. GAP is similar to a pre-loaded cash card backed by an insurance policy, and is presented at the time of service alongside your medical card. As you use the GAP, it is reducing your deductible with United Healthcare.
